Helpful Hints to Remember
Print clearly. Do not write over words or numbers. Cross out and rewrite.
Fill in all information, including the top portion. Person ordering permit, fax number, phone number, etc.
Double check your return fax numbers to make sure they work and have correct area code or put E-mail address down if you would like permit E-mailed back.
Fill out both license number and vin number or serial number. If you have no license number, put "NO PLATE" and fill in vin number or serial number.
Describe your transporting item:
unacceptable: boat excavator 200 Komatsu D8K
acceptable: 200 Komatsu Backhoe, Sea Ray Boat, Grove Crane, 140 G Cat Grader, Thuderbird Shovel
Mileposts are required, beginning and ending for each highway used, unless it is a monthly or a yearly request.
If you are not requesting weight on your permit, please leave Gross Weight and Legal Weight Capacity blank.
If overweight and no report #, you must answer all questions in overweight only section on the permit request sheet.
If no report # and overweight, please send to DOT for a report #.
If traveling on county or city roads, please give the number of miles travelling on those roads: CO miles.
Please sign and date permit requests.
Again, please write legibly, large and clear.
